What does the medical involve?
A taxi medical examination includes:
- Eye test
- Blood pressure check
- Review of your medical history
- Questions about medications you take
- General physical assessment
The doctor then completes the relevant medical form for your council’s licence application.
How long does a medical take?
Each medical usually takes around 15 minutes, but may take longer if you have a complex medical history
Where can I find the medical form?
Please select your licensing authority below:
- Cheshire West & Chester Council
- Knowsley Council
- Liverpool Council
- Sefton Council
- South Ribble Council
- St Helens Council
- Warrington Council (also bring D4 form)
- West Lancashire Council
- Wigan Council
- Wirral Council
- Wolverhampton Council
Alternatively, we can provide the medical form for you in clinic – please select this option when booking.
What do I need to bring to my appointment?
For all medicals, please bring:
- Photographic ID
- Proof of address (e.g. recent utility/phone bill, bank statement)
- Medication list
- Glasses/contact lenses (and prescription if available)
- D4/medical form (unless you have opted for forms to be provided in clinic during the booking process)
- Medical summary/records – these can be obtained via your GP practice, or through the NHS app. Some councils require full medical records rather than a medical summary – when booking your medical, please select your licensing authority to find out which type of medical records they require.
What happens after the medical?
After the doctor has completed the medical form, you will typically need to send a digital copy of the completed form to your licensing authority either by email or via an online portal.
We are able to provide a digital copy of your completed medical form after your medical – please select this option when booking.
How often do I need a medical?
Each council sets their own rules, so you should check with your licensing authority. However, for most councils:
First medical when applying for the licence
Every 5 years after age 45
Every year after age 65
